Practice Transformation Institute (PTI) an IACET-certified organization providing the health care community with continuing education and customized learning programs that improve patient health outcomes and the individual care delivery experience, has partnered with the Michigan Osteopathic Association (MOA) to create the MOA’s new Transformation Academy.
Practice Transformation Institute is Michigan’s leading provider of CME and IACET-accredited experiential learning programs for the patient-centered medical home (PCMH) and other primary care transformation initiatives.
PTI was founded in 2007 with the knowledge that a new kind of training was needed to implement the massive changes shaping the future of health care; holistic training that looks at multi-disciplinary care teams rather than a silo physician approach.
Transformation Academy, with classes offered through PTI, and at various locations identified as key by the MOA will provide web-based, blended and classroom style activities to MOA members. The new venture was prompted by the need for a comprehensive training curriculum on timely and emerging health care issues that provides interactive and engaging training sessions for busy health care professionals.
ATTENTION MOA PRIMARY CARE PHYSICIANS:
Michigan has been chosen by CMS as one of the 14 Comprehensive Primary Care+ (CPC+) regions nationally, and BCBSM’s proposal, along with Priority Health’s, has been provisionally selected within Michigan. For payers, the next step is to submit a Memorandum of Understanding to CMS by September 1, outlining their Agreement to the terms of CPC+ participation.
An in-person workshop that will solely focus on CPC+, including eligibility assessment and application assistance is scheduled for August 30, 2016 from 8:30AM - 1:30PM at the Troy Somerset Inn. June 22 – Diabetes Prevention Program (DPP) (Registration link) The good news is that people with pre-diabetes may delay the onset of type 2 diabetes and possibly return their blood glucose levels to normal by participating in a national program called the National Diabetes Prevention Program (NDPP). This evidence based lifestyle change program for preventing type 2 diabetes is being offered in Michigan communities and the Michigan Osteopathic Association has partnered with Medical Network One and the Practice Transformation Institute to facilitate workshops for your patients.
June 29 – Social Determinants of Health (Registration link) The social determinants of health (SDOH) are those factors that have an impact on health and well-being; the circumstances into which we were born, grow up, live, work and grow old. Although these factors may not directly cause illness, they are the root cause of ill health.
There is increasing evidence of the importance of addressing these root causes in order to improve the health of our communities. An understanding of the SDOH is especially important in primary care as we care and advocate for patients over time.
July 13 – The Nuts and Bolts of Transitions of Care (Registration link) Practice teams and physicians have long recognized the importance of communication in transition events.
Communication and documentation practices during transition are critical to providing safe, quality care for our patients. Doing transitions well is a major challenge for primary care physicians everywhere. Healthcare organizations, including DO's Together are tackling the issues by providing care management support, education, standardized practices and tools such as real-time ADTs including discharge diagnosis and medication reconciliation to capture necessary information.
July 20 – Developing Your Own Chronic Care Management Program (Registration link) Where do I start? Who is eligible? CMS finalized CPT 99490 as a monthly care management code with a national allowed amount of $42.60. This code provides payment for chronic care management services delivered telephonically by trained clinical support team members. This service is available to eligible Medicare patients. Chronic care management services require at least 20 minutes of clinical staff time directed by a physician or other qualified health care professional, per calendar month, with the several required elements. July 27 – STARS and HEDIS 2016 (Registration link) Physicians are held responsible for providing preventive and chronic care services to their patients. The National Committee for Quality Assurance’s (NCQA’s) Healthcare Effectiveness Data and Information Set (HEDIS) is a tool used by payers to evaluate performance on many aspects of health care services rendered by physicians. HEDIS consists of 75 measures across 8 domains of care and a tool by which payers determine the percentage of their members that are compliant with specific treatment protocols, such as ongoing activities to manage diabetes or other chronic conditions.
August 3 – Building a Practice Team (Registration link) How can team care contribute to improving efficiency in your practice? Can a physician’s organization provide team support in my practice? Who belongs on the primary care or specialists team? Hear how to identify the key staffing competencies necessary to support practice transformation and steps to begin the journey to patient centered medical home designation. Learn about a model to hire the right people for the right jobs to improve patient care in your practice. Tips and recommendations on how to build a team and develop a champion or leader to drive transformation in your medical practice.
August 10 – Why Should Pharmacists Join Your Practice Team? (Registration link) Pharmacists and pharmacy technicians already contribute to the health and wellness of individuals and communities by being the medication experts. The pharmacist’s role is expanding to provide leadership in quality improvement initiatives and medication-related measures. The ability to communicate and collaborate effectively as a member of the inter-professional practice team is essential to improve health outcomes and deliver safe and quality care to all patients.
September 14 – Beginning the Conversation of Advance Care Planning (Registration link) Voluntary ACP means the face-to-face service between a physician (or other qualified health care professional) and the patient discussing advance directives, with or without completing relevant legal forms. An advance directive is a document appointing an agent and/or recording the wishes of a patient pertaining to his/her medical treatment at a future time should he/she lack decisional capacity at that time. Voluntary ACP, upon agreement with the patient, would be an optional element of the Medicare Annual Wellness Visit (AWV). September 21 – Workflow Matters in Depression Screening (Registration link) The purpose of depression screening is early diagnosis and treatment. Screening tests are administered to people without current symptoms, but who may be at high risk for certain diseases or conditions. The US Preventive Services Task Force recommends that primary care doctors screen their adult patients for depression if they have a system in place to support care management and/or mental health treatment. There are a number of different questionnaires to screen for depression. Hear how one physician has integrated depression screening into his practice’s workflow. September 28 – Starting the Transformation Journey (Registration link) Listen to one physician’s journey toward Patient Centered Medical Home designation. Hear how physician’s organizations provide PCMH transformation support services and whether the support is provided onsite or remotely?
